Bank/sensor location note
Bank 1 is the cylinder bank containing cylinder No. 1. On an inline engine there is normally only one bank. Do not guess bank location from left/right orientation; verify the engine-specific service information.

Causes to test

- Unmetered air/vacuum leak downstream of the MAF sensor
- MAF/MAP measurement error or contaminated sensor
- Low fuel pressure or restricted fuel delivery
- Injector flow problem
- Exhaust leak ahead of an upstream oxygen/air-fuel sensor
- PCV, brake-booster, purge, or intake-manifold leak
Step-by-step diagnostic workflow
- Record fuel trims at idle and again near 2,500 RPM; the pattern helps separate vacuum leaks from fuel-delivery or measurement faults.
- Inspect intake ducting after the MAF, vacuum hoses, PCV connections, brake-booster hose, and intake manifold.
- Use a smoke test when appropriate to locate unmetered-air leaks.
- Check MAF/MAP plausibility and compare calculated load with expected engine operation.
- Verify fuel pressure/rail pressure under the condition that set the code.
- Check upstream exhaust leaks and sensor response before replacing an O2/A/F sensor.

Severity and drivability
Moderate. A persistent lean condition can cause poor drivability, detonation, elevated combustion temperature, or catalyst stress.
Stop driving and investigate immediately if there is a flashing MIL, oil-pressure warning, overheating, fuel leakage, severe misfire, major loss of power, abnormal mechanical noise, or a transmission that is slipping badly.

- Replacing oxygen sensors because a lean code mentions fuel mixture
- Cleaning or replacing the MAF without checking intake leaks
- Diagnosing from idle fuel trims only
